SMS Foundations

A Safety Management System (SMS) is a formal, top-down, organization-wide approach to managing safety risk and assuring the effectiveness of safety risk controls [14 CFR §5.3]. SMS is recognized by ICAO, the FAA, and civil aviation authorities worldwide as the standard for aviation safety management.

At Gold Aviation Services (Dolphin Atlantic Inc.), our SMS identifies hazards before they become incidents, manages risk systematically, and continuously improves safety performance across both the EMB-505 and EMB-550 operations.

Safety is accomplished through a continuing process of:

1. Hazard identification [14 CFR §5.51]
2. Safety risk management [14 CFR §5.53]

While eliminating accidents entirely remains the ultimate goal, the aviation system will never be completely free of hazards. As long as safety risks stay within an appropriate level of control, aviation operations maintain a balance between production and protection.

Many hazards arise from human factors. Common contributing factors include:

• Time pressures and workload
• Poor procedures and documentation
• Inadequate teamwork or crew briefings
• Poor fatigue management
• Lack of system knowledge
• Operator error in Human-Machine Interface

Gold Aviation Services (Dolphin Atlantic Inc.) identifies hazards proactively before they cause harm. The goal is to identify hazards at all levels of the organization and treat contributing factors so they do not produce an incident.

Primary hazard identification methods include:

• Voluntary hazard and incident reporting: Anyone can submit a safety report at any time through the FOS safety reporting tool.
• Investigation report review.
• Flight Risk Assessment Tool (FRAT).
• Internal and external audits.
• Safety performance data analysis.

Every SMS is built on four interrelated components [14 CFR §5.5]. All four must be present and functioning to have an effective safety program:

Safety Risk Management (SRM) is the formal, step-by-step process Gold Aviation Services (Dolphin Atlantic Inc.) uses to identify hazards and control risk [14 CFR §5.53]:

1. Describe the system: understand what operation or change is being assessed.
2. Identify the hazards: what can go wrong?
3. Assess and analyze the risk: how likely, and how severe?
4. Control the risk: implement mitigations that bring the risk to an acceptable level.

Once risk controls are implemented, their effectiveness must be monitored. If a control is not working, the SRM process begins again.

The goal is to eliminate the risk or mitigate it to an acceptable level, balanced against time, cost, and difficulty of mitigation. Risks should be managed to the lowest practicable level.

Safety Assurance (SA) assesses the safety effectiveness of Gold Aviation Services (Dolphin Atlantic Inc.) operations and enables continuous improvement by [14 CFR §5.71]:

• Monitoring, measuring, and reviewing safety performance indicators.
• Managing changes in the system through the Management of Change process.
• Continuously improving the safety system based on findings.

SA gives confidence that our SMS is meeting our safety objectives and that our risk controls are actually working and effective.

Safety Promotion covers training, communication, and safety culture [14 CFR §5.91]. It ensures all personnel understand their responsibilities and the organization's safety expectations.

Gold Safety — the SMS platform: Gold Aviation Services operates its SMS through Gold Safety, the company's integrated safety management platform. Gold Safety is where you submit safety reports, where your training assignments are dispatched and tracked, where NOTACs and NOTMXs are published, and where corrective actions are documented and monitored. Reporting confidentiality protections under 14 CFR §5.71 apply to all reports submitted through Gold Safety.
